Outreach Coordinator and Case Manager
The outreach coordinator and case manager will provide direct support and case management services to unhoused clients.
Specific duties include:
· Meeting clients in the jail and/or in open spaces
· Ensuring clients have the necessary vital documents for housing resources
· Doing CES enrollments and housing assessments in the jail and in outdoor spaces
· Coordinating care for clients with other community partners
· Overseeing all case management tasks related to unhoused clients
· Supporting the Program Director with housing navigation and housing related appointments.
Qualifications:
· A degree in social work or similar work experience
· Ability to use computers and Microsoft office
· Knowledge of the challenges specific to this populations
Pay is competitive and includes health insurance, PTO, sick pay, and a flexible schedule
